The downloadable forms published here must only be used in an emergency if your stock of red/white wallets has not arrived.

- one copy to be handed over at first departure airport

Opening and viewing a file
To open a file you wish to view, please double click with your left hand mouse button on the relevant file.

Downloading a file to your PC
To download a file to your own PC, please click the right mouse button on the relevant file and select 'Save Target As'

Printing a file or part of a file
Open the file as if you were going to view it. Once the file is opened, you will have the option to print one or all of the pages you see.
